

Injury Forms
The Process of the Injury fund is as follows:
When a player receives an injury they must fill in the appropriate form, also they must ensure to have a guardian sign (if juvenile), club secretary and finally send it to the county secretary who in turn will forward the form to Croke Park. (also include all receipts received) - this must be done within 8 weeks of the injury
*Important*
If the injury is going to take longer to heal than the two months and/or going to exceed €200 - then you must fill in the preliminary form first and this must be done within the eight weeks - this will cover you for up to a year after receiving injury and longer if the need arises

Physiotherapy sessions is limited to a maximum of 6 sessions.
If private treatment is needed then the player must ensure that permission is sought FROM CROKE PARK for private treatment beforehand

This includes
- Permission to see a Consultant – send Doctors letter to Head Office
- Permission for a MRI scan – send consultant letter to Head Office
- Permission for surgery – send results of MRI and consultant letter to Head Office

Injury Claim Form
ALL SECTIONS OF THE FORM ARE TO BE COMPLETED TO THE BEST KNOWLEDGE OF THE CLAIMANT. THIS FORM SHOULD BE COMPLETED IN BLOCK CAPITALS. THE COMPLETED FORM MUST BE FORWARDED TO HEAD OFFICE BY THE COUNTY SECRETARY. N.B Permission must be sought from the INJURY Fund Co-Ordinator for All Private Treatment prior to receiving the Treatment
